This workshop is presented by Shane Toohey, Podiatrist since 1981 and one of the founders of Artisan Orthotic Laboratory in 1990. He is a Clinical Senior Lecturer at the University of Western Australia. Shane gained his Graduate Acupuncture Diploma in 1991 and is a committee member of the Acupuncture Association of Australia, New Zealand and Asia.

It is the intention of this workshop that Podiatrists will immediately use acupuncture as one of their treatment modalities for many conditions commonly presented clinically.

For the effective fulfillment of this intention the number of participants in each workshop is limited. The particular venue will further influence the class size.

It is also intended that participants will use this workshop as a springboard to broaden their use of this modality and be keen to add further new skills to their therapeutic base.

The workshop begins with an explanation of the distinctions between Traditional Chinese Acupuncture and various modern forms of acupuncture. The evidence base for this modality and scientific perspective will also be discussed. Pre reading links will be provided.

The following topics are then discussed, keeping in mind that demonstrations of practical skills will be frequently used and hands on practice with partners will follow. The intention is for participants to be ready to use acupuncture immediately after the workshop in their first day back in clinic..

1.  Introduction to acupuncture needles and their safe use and overall safety considerations when using acupuncture.

2.  Point selection criteria.

3.  General considerations on methodology of use.

4.  Comprehensive list of commonly used points  including the point characteristics.

6.  An introduction to the use of  what has been termed ‘dry needling’ in the treatment of myofascial trigger points.

7.  Outline of point combinations for common condition including using a combination of acupuncture and dry needling.

8.   A discussion and outline of methods of combining acupuncture into your overall treatment protocols along with your current modalities.

The workshop manual and pre-reading information is fowarded before the commencement of the workshop.

The Advanced workshops will continue with hands on practical sessions. After a review of the content from the base workshop this workshop will focus on specific techniques used for working through the complex management required for dealing with chronic pain. You will be taught the 'Superficial Dry Needling', (Baldry) and a Zonal approach (Mark Seam).

'Dry needling' is generally a western medical technique that is used in isolation. When musculo skeletal problems are chronic they usually become very complex with 'holding patterns' (protective) that cause the dysfunction and pain to spread within the sytem. The workshop shows advanced methods of dealing with these complex presentations.
When you look at the flow of the connections between the fascial planes you will see how there is an unbroken connection between parts of the body along the fascial planes. If you then look at a pain chart from your client showing where their areas are you will often see'patterns of connection.'. Specific patterns have been identified, called 'zones' by Mark Seam. Mark has developed formulas of traditional points that can be used for each zone that will help 'open' the zone for more effective dry needling. These zones and formulas are presented in this workshop. There are some new traditional points to learn to put the zonal approach into action!

When pain is widespread and sensitivity is high then dry needling can become very hit and miss as 'everything is tender'. Also individuals may vary incredibly in their responsiveness and what is OK for one person may be unneccesarily painful in another. Baldry has developed a technique called 'superficial dry needling' where extensive but very shallow and quick dry needling may be used to reduce sensitivity and isolate the 'real' triggers. An extreme example is fibromyalgia, but this technique gives you a tool to provide real help in this condition.I find this technique is also a great way to begin any needling treatments on children. The more sensitive the presentation the more likely I am to use this technique.

These techniques, of course, may be combined. As usual I would also always advise honouring your podiatric skills and treat any mechanical deficiencies that may be a factor in the persistence of stress in the affected tissues to be treated concurrently. If you have skills in manual therapies then I would also expect them to be used as well in releasing any 'blockages' in joint ranges of motion.Such an integrated approach may lead to the resolution of symptoms in someone who otherwise would continue to det eriorate in the normal system and 'slip through the cracks'.

We will look at a couple of scenarios of common complex conditions eg severe long term shin splints and 'plantar fasciitis' as an example of a really mixed technique approach.

In most venues this workshop is usually booked out before the end of the early bird deadline.

Advanced Orthotic Therapy

Workshop Outline

Orthotic therapy as it has been paracticed for the past few decades is based on the theories shown by research to be fundamentally incorrect.  More recently, simplified practical systems have become more widely used.  These systems have even less evidence base and rely often on the gullibility of the practitioner and general public.  If this is the context within which we practice, on what basis can we deliver mechanical interventions.  As Podiatrists we are well aware of how effective mechanical interventions can be in previously unresponsive conditions and also how sometimes our orthoses are completely ineffective and even a source of exacerbation of the presenting problem.  The possibility is that our interventions work and don't work for reasons outside our theories.  We'll look at a system for treating each case on its separately rather that trying to create some teoretical normality.

This intensive one day workshop presents a logical and practical assessment and diagnostic process, including the use of chairside devices.  Chairside devices may stand alone as the mechanical intervention of choice or become the basis for effective prescribing of customised devices.  The participants are introduced to an extensive range of cast and device variables.  All cast variations are explained and demonstrated in their practical methododlogy such that their functional effect is easily understood.  This will help participants understand also how scanning systems pretend to deliver the same effect and fall short.  The workshop concludes with practical sessions on casting, trouble shooting and other aspects of case management.  Sections of this workshop are conducted with particicpnats working in pairs.

This workshop will be conducted by Shane Toohey, Managing Director of Artisan Orthotic Laboratory.
